A high BMI in pregnancy is associated with greater risk of a variety of pregnancy-related complications compared with women within a normal BMI. BMI is not always accurate, however we are referring to those with a high body fat relative to their height. 

Some of the (evidence based) proven risks of obesity in pregnancy include pre-eclampsia and gestational diabetes. Pregnant women who are clinically obese are also at increased risk of caesarean birth. Maternal size can make the assessment of fetal size, presentation and external monitoring of fetal heart tracing more challenging during pregnancy and labour. 

 

So does that mean these women have no choice?

Absolutely not!

We live in a risk adverse world. Our maternity system is all centred around risk. However, every family has the right to informed choice. I encourage you all to do your research about your options and balancing the pros and cons for each procedure relative to your unique pregnancy.

You do not HAVE to do anything. Women with a higher BMI can have the birth experience of their choice. You just may need to fight for it more than others. Lean on supports to get well informed. Take a private antenatal course with us or hire one of our doulas to support you every step of the way.

 

So what about weight gain in pregnancy?

The RCOG says “There is a lack of consensus on optimal gestational weight gain. Until further evidence is available, a focus on a healthy diet may be more applicable than prescribed weight gain targets.” This means you should not be bullied or shamed as you gain weight in pregnancy by any health care staff. Aiming for a healthy balance in your day to day life will help you to stay positive and give you the best opportunity for a healthy pregnancy and a positive birth experience. 

  • Try to choose nutritionally dense, healthy food. 

  • Go for a walk each day

  • Gentle exercise daily, such as pregnancy yoga

  • Focus on your mental well-being too (mindfulness, GentleBirth, HypnoBirthing, Headspace App, Counselling etc)
